12-month introductory professional consultation fees
October 19, 2026–October 18, 2027 · Eligible self-pay visits · Professional consultation only
12-month introductory self-pay professional consultation fees, where permitted: $75 for an initial assessment and $50 for a follow-up consultation. Available for eligible visits occurring October 19, 2026 through October 18, 2027. Outside this promotional period, standard professional fees are $150 initial and $100 follow-up. Fees cover professional consultation only; medications, laboratory tests, imaging, procedures, specialist consultations, and other outside services are additional. These are per-visit fees, not insurance copayments or a subscription. Confirm eligibility and payment arrangements before booking. If insurance is used, patient responsibility follows the applicable payer contract and program rules.
This is one fixed 12-month promotional period, October 19, 2026–October 18, 2027, inclusive, based on the date of the visit in Chicago time. It is not a separate 12-month discount starting when each patient enrolls. Standard fees apply before October 19, 2026 and from October 19, 2027: $150 initial assessment and $100 follow-up. Follow-up pricing also applies to a separately scheduled treatment-planning consultation. No prepaid package, monthly membership, or added participation fee is required. Existing written fee agreements remain in effect unless new terms are agreed; the offer does not change past bills or automatically change existing appointments. Confirm the applicable fee before your visit.

The professional consultation may include weight and health assessment, nutrition and activity counseling, discussion of behavioral goals, and medication evaluation or monitoring when appropriate. The clinician determines what can be addressed at the visit. Medication is not included or guaranteed.
Laboratory and diagnostic testing, medicines, pharmacy charges, procedures, specialist consultations, and services from outside providers are not included. Confirm separate charges with the provider delivering those services.
If you have insurance
These self-pay rates are not a quote of your insurance copayment, deductible, or coinsurance. When a service is billed to insurance, the applicable payer contract and program rules determine patient responsibility; the practice does not add the self-pay visit fee to that same insurance-billed service. Medicare, Medicaid, and other program billing arrangements must be confirmed before a self-pay option is offered.
Confirm the service, total expected charge, payment timing, and any financial agreement before your visit. This website does not collect payment details or authorize automatic charges. A public fee list does not replace an individualized Good Faith Estimate when one is required.
Insurance participation
Insurance network participation has not been verified on this website. Contact the practice and your insurer to confirm whether your clinician and visit are in network, whether authorization is needed, and your expected responsibility. No insurer acceptance or reimbursement is guaranteed.
Our services
Services include nutrition, physical activity, and behavioral counseling; evaluation for medication treatment and monitoring when clinically appropriate; and video consultations. Treatment is individualized. A prescription is not guaranteed, and some needs require in-person assessment or referral.
Who can receive care?
Based in Illinois. We welcome inquiries for patients of all ages. Before booking, the practice must confirm that care is available for the patient’s age, physical location, clinical needs, and the clinician’s applicable licensure or authorization. This website does not establish nationwide treatment availability. A parent or legal guardian must be involved in care for minors as applicable.
Your right to a Good Faith Estimate
If you do not have insurance or are not using it for your care, you generally have the right to receive a written Good Faith Estimate of expected charges when you request one or schedule care at least three business days in advance. Contact the practice to request an estimate before your appointment.
You may be able to dispute a bill from a provider if it is at least $400 more than that provider’s estimate. Keep a copy of your estimate and bill.
