Book your first
weight-care consultation.
Start with a physician-directed conversation about your health, goals, and treatment options. Follow these three steps to arrange your video visit.
Carepatron is an external booking service. Confirm eligibility and costs before reserving a visit. Viewing this page or completing forms does not book an appointment.
Three steps to get started.
- Confirm your care and costs. Our practice is based in Chicago, Illinois. For eligible self-pay visits from October 19, 2026 through October 18, 2027, introductory professional consultation fees are $75 initial and $50 follow-up. Outside that period, standard fees are $150 initial and $100 follow-up. These cover professional consultation only; medications, laboratory tests, imaging, procedures, specialist consultations, and other outside services are additional. Contact us to confirm care for your age and physical location, payment arrangements, and any insurance questions. These fees are not insurance copayments. Network participation is not verified on this website. Medicines, testing, and outside services may cost extra. Full fee information and estimate rights ↗
- Choose your appointment. Open Carepatron appointment availability ↗. Select the appropriate visit and follow its booking instructions. Keep the confirmation, appointment time, and video connection details. A booking request or acknowledgment is not necessarily a confirmed appointment. Look for the confirmed visit date, time, time zone, and video connection instructions. If these are missing, check spam/junk and contact the practice before assuming your visit is booked. Automated reminders depend on the account settings and plan; do not rely on receiving a reminder.
- Prepare after booking. Download your patient packet ↗, complete the requested intake and consent documents, and return them through your practice-linked Carepatron account using the secure return instructions ↗. Have your medication list and questions ready.
What your first visit covers.
Discuss your weight history, medical conditions, medicines, eating and activity patterns, and goals. Explore individualized options and agree on next steps. Medication is prescribed only when appropriate; a consultation does not guarantee a prescription.
See the first-visit guide ↗Questions after requesting an appointment?
We aim to respond to administrative calls and messages within two business days (Monday–Friday, excluding holidays). This is a response target, not a guaranteed callback time. If you have not heard back after two business days, call (312) 952-6720. For emergencies, call 911; do not wait for a reply.
Appointment confirmation and an administrative callback are different. A reply about scheduling does not guarantee that a slot has been reserved. The practice must confirm your appointment details.
Appointments that fit your schedule.
Practice hours: Monday–Friday, 7–8 p.m. Central Time, and Saturday, 8–11 a.m. Central Time. Closed Sunday. Available appointments are shown in Carepatron; hours do not guarantee an open slot.

Please give at least 24 hours’ notice.
Please cancel or reschedule at least 24 hours before your scheduled appointment. Call (312) 952-6720, or use the cancellation/rescheduling option in your Carepatron appointment confirmation, if available.
If you need to cancel within 24 hours or miss your appointment, contact the office to arrange next steps. This policy does not establish a cancellation fee or authorize an automatic charge. Any fee must be disclosed and agreed separately in advance.
Read or download the cancellation policy ↗
Effective October 17, 2026. The 24-hour notice period applies consistently to practice policies and any conditional payment authorization. If an older form shows a different notice period, contact the office before signing it.
BEFORE YOUR APPOINTMENT
Your forms, in one place.
Open the patient packet for intake, telehealth consent and practice policies. Records release is separate and used only when requested. Saving a PDF does not send it to the office. See how to return completed forms through your Carepatron portal.
Open the patient forms hub ↗A free reminder on your own device.
Have a confirmed appointment? Add it to your own calendar with optional alerts. No account or subscription is needed. This does not send email or SMS, or sync appointment changes.
