Patient intake and consent packet
Welcome instructions, three history pages, treatment and video-visit consent, and practice policies. Ask questions before signing consent. The privacy acknowledgment is separate and should be completed only after receiving the full notice.
Obtain the complete practice Notice of Privacy Practices from the office before acknowledging receipt. The acknowledgment is not the notice itself. For children, the office confirms guardian authority, age-appropriate services and applicable confidentiality rights. Internal triage protocols and lab checklists are not patient consent forms.
12-month introductory self-pay professional consultation fees, where permitted: $75 for an initial assessment and $50 for a follow-up consultation. Available for eligible visits occurring October 19, 2026 through October 18, 2027. Outside this promotional period, standard professional fees are $150 initial and $100 follow-up. Fees cover professional consultation only; medications, laboratory tests, imaging, procedures, specialist consultations, and other outside services are additional. These are per-visit fees, not insurance copayments or a subscription. Confirm eligibility and payment arrangements before booking. If insurance is used, patient responsibility follows the applicable payer contract and program rules.
Based in Illinois. We welcome inquiries for patients of all ages. Before booking, the practice must confirm that care is available for the patient’s age, physical location, clinical needs, and the clinician’s applicable licensure or authorization. This website does not establish nationwide treatment availability. A parent or legal guardian must be involved in care for minors as applicable.
