Usual meals, drinks, portions, eating times, food access, emotional eating, and a typical day’s food intake.
Activity, sedentary time, mobility or pain, sleep duration, snoring, and daytime fatigue.
Conditions such as diabetes, high blood pressure, sleep apnea, liver disease, or joint pain; mental wellbeing, stress, and eating concerns.
Pregnancy status or plans, when relevant, and medicines that may affect weight or treatment safety are also important. Share medical details through the approved clinical forms and consultation.
Your clinician may review height, weight, BMI, waist circumference, blood pressure, pulse, and other measurements. Telehealth can limit parts of an examination; in-person assessment may be recommended.
Testing may include glucose or A1c, cholesterol, and kidney or liver function when appropriate. Thyroid tests, vitamin levels, pregnancy testing, sleep studies, or other investigations depend on your history and clinical findings. This is not a standard laboratory order, and every participant does not need every test.
Do not arrange extra tests or change medicines based on this checklist. Ask the clinician what is appropriate and confirm any separate testing charges.
A 5%–10% weight-change goal over about six months can be a starting discussion for some adults, rather than a promise or a deadline. Food intake, activity, and treatment goals are individualized; adult targets do not apply to children.
Before ending the visit, confirm your next appointment, any tests or referrals, what to monitor, and how to contact the practice. Follow-up and medication adjustment may be needed within 2–4 weeks or sooner depending on your care; laboratory monitoring is based on clinical need.
Visit length and follow-up timing will be confirmed by the practice. Contact the clinical team promptly about treatment concerns; the public website and automated guide are not monitored clinical services.
Call the practice for assistance. Use email only for administrative questions; do not email completed medical forms or sensitive information.
